Equiti ("Company") seeks your voluntary consent for participation in certain media-related and marketing activities. Your participation may include but is not limited to:
- Video Interviews – Sharing your experiences and perspectives about our organization and events associated with our organization.
- Videos – Appearing in videos based on your attendance at events associated with our organization.
- Photography – Appearing in photos based on your attendance at events associated with our organization.
- Marketing Purposes – Using the above media for promotional, advertising, and marketing initiatives, including digital and print materials, social media, websites, and other public platforms. (collectively as "Content")
By registering for the 2026 Language Access Leadership Symposium, you acknowledge and voluntarily agree to the following:
- I understand that my participation is completely voluntary and that I may decline or withdraw consent at any time by providing written notification to marketing@equitihealth.com.
- I grant Company the irrevocable right to use, reproduce, publish, distribute, and modify the recorded Content in connection with marketing and promotional efforts.
- I understand that these materials or Content may be publicly accessible and that I will not receive any compensation for my participation.
- I release Equiti Health, LLC, its representatives, employees, and affiliates from any claims related to the use of my likeness in the approved materials.
- I understand that my participation is voluntary and that I may opt out or revoke my consent at any time, even after registering, by submitting a written request to marketing@equitihealth.com. Upon receipt of such request, Equiti will make reasonable efforts to cease future use of my likeness in unpublished materials. However, I acknowledge that any materials already publicly disseminated prior to the opt-out request may not be retracted.
